AGREEMENT

By submitting this form, I certify that I have read and understand the quotations and the consumer information on the back of this quotation. I agree to remove the pesticide notification signs that will be posted on my property no sooner than 24 hours following the application and no later than 72 hours following the application. I authorize Royal Turf to service my lawn. They are to continue servicing my lawn without further notification or authorization. I understand that I may terminate this agreement at any time by simply making payment for the last work done and notifying Royal Turf by mail that I do not wish to continue service. Terms net 10 days. 1 1/2 % after 30 days. Client will be responsible for all fees incurred collecting unpaid balances.
